Which

Certified copy of the parents’ Marriage Certificate (except in cases when they are registered in Nis or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office); certified copy of the Citizenship Certificate of one parent (for a person registered in City of Nis General Register Office and Evidence of Citizens, the certificate should be obtained ex officio); valid ID cards of the parents; mother’s Birth Certificate ( if she is not married and if she is registered in Nis and in municipalities/cities which are the part of the Republic of Serbia Central Registry Office); green card (original document) for IDPs.

Other documents (if the parent is of foreign nationality): certified copy of the passport; Certificate of Free Marital Status; Birth Certificate (international form) or duly legalized and translated document by authorized court interpreter.

Citizenship Certificate (original document or certified copy) issued within 6 months at the most   (for person registered into City of Nis General Register Office and Evidence of Citizens, the certificate is obtained ex officio); Certificate of Residence (original document) issued by Nis Police Department; Birth Certificate, issued within 6 months at the most.

Note 1: for a person born in Republic of Slovenia it is necessary to submit International Birth Certificate (issued within 6 months at the most); Marriage Certificate – original document or certified copy (for a person registered in City of Nis Register of Marriages or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, the certificate is obtained ex officio); evidence on paid fee in the amount of 2.130,00 RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying the fee: Republic administrative fee).

Other: certified copy of final court ruling on the divorce; Death Certificate for the spouse (original document of certified copy); certified copy of the custody papers (issued by respective authority).

Note 2: for a person registered in the City of Nis Evidence of the Deceased, or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, the certificate is obtained ex officio.

Birth Certificate and Marriage Certificate – original documents or certified copies (if the applicant is registered in City of Nis General Registry, or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, he/she does not submit the certificates, they are obtained ex officio); Citizenship Certificates for parents (if the parents are registered in the City of Nis Registry and Evidence of Citizens, the certificate is obtained ex officio); certificate that child has not been registered in the General Registry where he/she was born; report on birth (original document) issued by the medical institution where the child was born or statements by two witnesses who were present at the child ‘s birth; certified copies of the parent’s  and witnesses’ ID cards; statement made by the parent of the child on the date and place of the child’s birth; consent statement by the child over 10 years of age with  chosen personal name; records on the recognition of paternity (original document) (if the parents of the child are not married); record on selected personal name for the child with the statements from the parents on date and place of child’s birth ( if the parents are married); Report from Forensic Medicine Institute about child’s age and gender; a certified copy of the health insurance card (optional); records on the vaccination or immunization; report card from the school; some  child’s personal or other document from the period immediately prior to the submission of the application (if applicable); evidence on paid fee in the amount of 700,00 RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying the fee: republic administrative fee).

Birth Certificates of the future spouses (except in cases when they are registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office); Citizenship Certificates of the future spouses (except in cases when they are registered in  City of Nis General Registry, then the certificates are obtained ex officio); marriage license for a minor issued  by the competent court (in cases when the minor is getting married); valid ID cards of the future spouses; evidence on paid fee in the amount of  400,00 RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying the fee: republic administrative fee).

Note: the future spouses, who were already married, should submit the proof on termination of the previous marriage, if it has not been previously recorded (final court ruling on the divorce, marriage annulment, or Death Certificate for the deceased spouse).

Birth Certificate of a foreign national in the international form or duly legalized and translated document by a certified court interpreter; Certificate on Free Marital Status of a foreign national 

in the international form or duly legalized and translated document by a certified court interpreter; valid passport.

Application for registering the marriage concluded abroad (by verbal report or in writing); Marriage Certificate in the international form or duly legalized and translated document by a certified court interpreter; Republic of Serbia Citizenship Certificate, issued within 6 months at the most (in case a person is not registered in City of Nis Registry, Certificate of Residence is issued by respective Police Department).

Application for issuing the certificate of free marital status 1) of general type (the certificate proving that the applicant is not registered into Registry of Marriages, b) ( the certificate that the applicant is not married and that are not any marital impediments to concluding the marriage with a particular person before the competent authority abroad); Birth Certificate of  foreign national in the international form or duly legalized and translated document by a certified court interpreter; Citizenship Certificates (except in cases when the applicant is registered in Nis, then the certificate is obtained ex officio); evidence on paid fee in the amount of  1050,00 RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying the fee: republic administrative fee).

Note: Certificate of Free Marital Status is issued upon personal request according to the place of registering in the Registry of Births. If the application is submitted by the person whom the document does not refer to and is not a part of the family it is necessary to submit the authorization.

Child’s Birth Certificate (if the child is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, the document is obtained ex officio); Birth Certificate for the child’s mother (if she is registered in City of Nis General Registry, or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office); statement on recognition of paternity; valid ID cards of the parents.

Note: Birth Certificate of the child’s mother is needed when it comes to the recognition of paternity of the child who has not been registered in the Registry of Births within the period of 30 days.

A certified copy of the Birth Certificate (if the applicant is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office,  the document is obtained ex officio); copy of ID card or the print out of the biometric ID card; certified copy of the Marriage Certificate (if the applicant is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, the document is obtained ex officio); certified copy of the Citizenship Certificate, issued within 6 months at the most (the applicant registered in City of Nis General Registry and Evidence of Citizens does not submit the document which proves that there are not any criminal proceedings against the applicant for the offense that is prosecuted ex officio; such document is obtained ex officio by Basic Court or by Basic or Higher Persecutor’s Office); certified copy of the final court ruling on the divorce of marriage (optional); Certificate of Residence issued by Nis Police Department; evidence on paid fee in the amount of  700,00  RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying: the republic administrative fee).

Certified copy of the child’s Birth Certificate (if the child is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, does not submit this document which obtained ex officio); copies of the parents’ ID cards; Marriage Certificates of the parents (if the parents are registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, does not submit this document which obtained ex officio); certified copy of the child’s Birth Certificate (if the child is registered in City of Nis General Registry and Evidence of Citizens does not submit the document; it is obtained ex officio; consent statement by the child over 10 years of age about the change of name (it is necessary for a child to be present); consent from the parents for the change of the child’s personal name (it is necessary for the parents to be present); original document issued by the Higher Court that there are not any criminal proceedings against the minor; Certificate of Residence issued by Nis Police Department; certified copy of the final court ruling on the divorce of the marriage ( in case when the parents are divorced); evidence on paid fee in the amount of  700,00  RSD (paid into account No 840-742221843-57, reference No 97 05 – 128, Recipient: Republic of Serbia, the purpose of paying the fee: republic administrative fee).

Note: If necessary, the respective authority may request other documents – certified copy of the child’s school report card or child’s passport, certified copy of the health care insurance card, Death Certificate (in case one of the parents is deceased), certified proxy for representation, statement of approval of the change of child’s personal name (in cases when one parent is absent), record on the recognition of paternity, etc.

Death Certificate (original document) (issued by the relevant medical institution or by the doctor in charge of determining the time and cause of death for those who die outside medical institution); Birth Certificate of the deceased (if the deceased is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, this document is obtained ex officio); ID card of the applicant; ID card of the deceased (original document); Marriage Certificate of the deceased (original document or certified copy – this is optional, if the deceased is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, this document is obtained ex officio); Citizenship Certificate (original document or certified copy) issued within 6 months at the most (if the deceased is registered in City of Nis General Registry and Evidence of Citizens, the document is obtained ex officio).

Note: Beside stated above, the applicant may be requested to submit the Marriage Certificate of the deceased (if Birth Certificate of the deceased does not contain information on marital status); Birth Certificate of the spouse of the deceased (if Birth Certificate of the deceased does not contain information on marital status); Death Certificate of the spouse of the deceased (if Birth Certificate of the deceased does not contain information on marital status).

Application for registering the death that occurred abroad (by verbal report or in writing); Death Certificate in the international form or duly legalized and translated document by a certified court interpreter; Certificate of the Residence of the deceased (original document or certified copy); Birth Certificate of the deceased (original document or certified copy – if the deceased is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, this document is obtained ex officio); Marriage Certificate of the deceased (optional and if the if the deceased is registered in City of Nis General Registry or in municipalities/cities which are the part of the Republic of Serbia Central Registry Office, this document is obtained ex officio); Citizenship Certificate of the deceased (if the deceased is registered in City of Nis General Registry and Evidence of Citizens, the document is obtained ex officio).

Note: Besides stated above the applicant may be requested to submit the Marriage Certificate of the deceased (if Birth Certificate of the deceased does not contain information on marital status); Birth Certificate of the spouse of the deceased (if Birth Certificate of the deceased does not contain information on marital status); Death Certificate of the spouse of the deceased (if Birth Certificate of the deceased does not contain information on marital status).

Certified copy of the identity card; a certified copy of the document indicating the accuracy of the data that need to be corrected (birth certificates, proof of citizenship, etc).

EXAMPLES:

In order to correct this mistake, please submit your Birth Certificate or the basic data entered when registering (name and surname, date and place of birth, the names of the parents), Birth Certificate of the father and parents’ Marriage Certificate (if the parents are registered in Nis, please submit just the registering data without actual certificates).

In order to change the surname in this particular case, the following documents are needed: Birth Certificate, Marriage Certificate, Citizenship Certificate (if you are not registered in Nis), the document issued by Basic Court or by Basic or Higher Persecutor’s Office that there are not any criminal proceedings against you, identity card (certified copy of print out of the biometric ID).

If you got married abroad, first, it is necessary to register this fact into Register of Marriages in Nis, which can be done through Serbian Embassy or nearest consulate. It is also necessary that the ruling on the divorce of marriage made by foreign court is confirmed by our court. After that, you apply for the change of surname by giving the proxy to the person who will apply instead of you or by giving the proxy to the lawyer. This proxy, power of attorney, should be certified at the Embassy or consulate of Republic of Serbia and it should have clear statement of the change you want to made.

The following documents of the deceased should be submitted: Birth Certificate, Marriage Certificate (if she was married), Citizenship Certificate and ID (certified copy or the print out of the biometric ID). If she was registered in Nis for the above stated documents it is not necessary to submit the actual certificates; the basic data needed for registering will be  sufficient such as: name and surname of the deceased, date and place of birth, the names of the parents and alike.

Which documents are needed for registration of the birth into Register of Births for person born on the territory of some of the former SFRY?
